Job Type: Full-Time

Job Pay: $75K - $100K per year+ productivity bonuses

Job Location: Crandon WI

Responsibilities:

• Evaluate patients and create individualized plans of care
• Providing 1 to 1 patient care
• Deliver hands-on, patient centered treatment and education
• Monitor progress and adjust care plans to achieve meaningful outcomes

Qualifications:

• Licensed Physical Therapist (or ability to obtain licensure in the state of Wisconsin)
• Passion for evidence-based, patient centered care
