Physical Therapist - Travel Contract

Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association or equivalent accrediting bodies. Current, active, and unencumbered Florida physical therapy license required prior to the start of assignment. Minimum 2-3 years of clinical physical therapy experience preferred as indicated by the client; candidates with equivalent experience will be considered. (Preferred) Current BLS/CPR certification from American Heart Association or American Red Cross may be required. Experience providing skilled physical therapy in home health or outpatient home visit settings preferred. Demonstrated ability to perform patient evaluations, develop and update plans of care, and document clinical progress in electronic health records. Valid driver’s license, reliable transportation, and agency-required liability insurance for performing home visits. Ability to supervise and evaluate PTAs and home health aides as assigned and to coordinate care with physicians and interdisciplinary team members. Proficiency with basic computer functions including email, internet navigation, and electronic documentation systems. Assignment Details Facility Type: Home Health Shift Name: Evenings Schedule Pattern: 5x8 (five shifts per week, eight-hour shifts) Scope of Practice: Perform patient evaluations, provide therapeutic services per physician orders, and participate in plan of care development and discharge planning. Supervisory Responsibilities: May directly supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned. Travel/Float Requirements: Must have reliable transportation for home visits within the assigned service area.
