We’re seeking a dynamic Director of Post-Acute Partnerships to expand referral partnerships, strengthen hospital relationships, and drive census growth across our skilled nursing centers. This is an ideal opportunity for a clinically knowledgeable healthcare professional who enjoys networking, strategy, and making a measurable difference in patient care. What You’ll Do Develop and strengthen relationships with hospitals, physicians, case managers, discharge planners, accountable care organizations, and community partners. Identify new referral opportunities while maintaining and growing existing partnerships. Coordinate patient referrals from initial clinical review through admission, ensuring seamless transitions of care. Collaborate with admissions, nursing, and executive leadership to deliver outstanding patient and customer experiences. Promote the clinical capabilities and specialty programs that set our centers apart. Leverage CRM technology and market data to identify growth opportunities and measure performance. Create and execute strategic business development plans that increase admissions and market share. Analyze referral trends, conversion metrics, and market intelligence to continuously improve results. Qualifications Preferred qualifications include: RN or LPN license preferred (where applicable), but not required. Valid driver’s license with the ability to travel locally. Five or more years of clinical, healthcare sales, business development, case management, or post-acute care experience. Experience working with hospitals, managed care organizations, or skilled nursing facilities is highly desirable. Excellent communication, presentation, and relationship-building skills. Ability to analyze data, develop strategic plans, and achieve measurable performance goals. Posted Salary Range USD $70,000.00 - USD $85,000.00 /Yr.
