Clinical External Liaison – Danville & Williamsport, PA

We are looking for a Clinical External Liaison to join our team and help build strong relationships with hospitals, physicians, case managers, social workers, and other referral sources throughout the Danville and Williamsport areas.

This is a great opportunity for a motivated healthcare professional who enjoys building relationships, growing referral networks, and representing post-acute care services in the community.

Key Responsibilities

• Develop and maintain strong relationships with hospitals, physicians, case managers, social workers, and community referral sources.
• Increase awareness of our skilled nursing and post-acute services.
• Identify new referral opportunities and help grow census.
• Conduct facility visits, meet with referral partners, and represent our organization professionally.
• Work closely with admissions and clinical teams to ensure a smooth referral process.
• Track referral activity and maintain regular communication with referral partners.
• Attend community events, healthcare meetings, and networking opportunities.

Ideal Candidate

• Healthcare, sales, marketing, business development, or related experience preferred.
• Experience with skilled nursing, post-acute care, hospitals, rehabilitation, or healthcare referrals is a plus.
• Strong communication and relationship-building skills.
• Self-motivated, organized, and comfortable working independently.
• Must be comfortable traveling throughout the Danville and Williamsport markets.
