Job Summary We are seeking a qualified Nurse Practitioner specializing in Palliative Care to serve patients throughout the Hampton/Allendale area. The provider will deliver exceptional patient care across multiple settings, including homes, facilities, and telehealth, and collaborate with a multidisciplinary team to ensure comprehensive patient management. Employment & Compensation This is a full‑time salary‑based position with monthly bonus opportunities. Shift options: 8‑hour (Mon‑Fri 8:00–5:00) or 10‑hour (Mon‑Thu 7:00–6:00). Coordinate care activities with the interdisciplinary team to ensure continuity and optimal patient outcomes. Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members. Participate in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination. Foster effective communication and collaboration to facilitate seamless patient management. Meet staff efficiency expectations while providing high‑quality care in homes, facilities, telehealth, clinics, and specialties. Document care management activities accurately to support patient care plans and regulatory compliance. Provide education and resources related to patients’ conditions, treatment plans, self‑management, and preventive care. Offer support and guidance to patients and families to enhance understanding of healthcare options. Adhere to all relevant healthcare regulations, including HIPAA and Medicare guidelines. Monitor quality measures and gaps in care, identify opportunities for improvement, and implement corrective actions. Qualifications Valid Nurse Practitioner, Physician Assistant, or Physician license in good standing in the state of employment. Experience in patient care, preferably in home health, clinic, facility, telehealth, or specialty setting. Strong communication, collaboration, and interpersonal skills for team environments. Ability to evaluate high‑risk patients, develop care plans, and coordinate care activities. Commitment to compassionate, patient‑centered care and a culture of excellence. Strong organizational and time‑management skills, able to multitask and prioritize. Strong written and verbal communication skills. Basic computer proficiency. Leadership skills and ability to foster a cooperative work environment. Team player who can manage multiple responsibilities and exercise sound judgment. Flexibility to work varied hours, travel between offices and facilities, and hold a valid driver’s license with insured vehicle.
