Administer medications and therapies as ordered, document administration, and monitor for adverse reactions. Develop and update individualized care plans, coordinate with the interdisciplinary team, and communicate changes to physicians. Perform patient and family education, discharge planning, and coordination of care with external providers as appropriate. Maintain accurate, timely documentation in the electronic health record and ensure compliance with safety, infection control, and privacy policies.
