Remote Rural Health Initiative Lead: ROOTS Hub 2

Vaya Health is seeking a ROOTS Hub Initiative 2 Lead to drive implementation and oversight of Initiative 2 within NC's Region 3. The role partners with healthcare providers, public health agencies, FQHCs, and community organizations to improve rural health access and outcomes.

Responsibilities include coordinating prevention, chronic disease management, maternal health, and nutrition initiatives, tracking milestones, and reporting to NC DHHS requirements.
