LPN – Contract to Perm (Long-Term Care)

Location: NY, 14456
Contract Dates: May 12, 2026 – August 15, 2026 (13 weeks )
Pay: $1,136 – $1,285/week

Schedule:

• Full-time: 40 hours/week

• 5x8-hour shifts (12-hour clock not applicable – 8-hour shifts)

• Days & evenings (scheduled as needed)

• Every other weekend required (EOW)

• Holidays required

• Must begin orientation on Tuesday or Thursday

Position Summary & Responsibilities:

• Provide direct and indirect nursing care under RN supervision

• Monitor and document patient/resident physical and psychosocial status

• Communicate findings and updates to RN and interdisciplinary team

• Develop and revise individualized care plans with care team input

• Support geriatric residents with age-appropriate, safe care

• Participate in staff orientation, mentoring, and unit support activities

Requirements:

• Active New York LPN license required

• BLS (AHA or RQI only) required

• Minimum 1 year LPN experience in LTC setting required

• Strong knowledge of geriatric and adult patient care required

Additional Details:

• Contract-to-perm opportunity

• Must demonstrate understanding of lifespan development and age-specific care

• Exposure to bloodborne pathogens possible (standard clinical risk)

• Works under RN direction with interdisciplinary collaboration
