Job Description Position Overview\n Community Care Physicians (CCP) is seeking a Nurse Practitioner or Physician Assistant to join Community Care Internal Medicine Clifton Park, an established adult primary care practice located within the Clifton Park Health Park. \n \n This opportunity offers the ability to work as part of a close-knit team consisting of an experienced Internal Medicine physician and two advanced practitioners dedicated to providing personalized, evidence-based care for adult patients. The practice emphasizes preventative medicine, chronic disease management, care coordination, and building long-term patient relationships. Patients are active participants in their care, supported by a team committed to communication, accessibility, and high-quality outcomes. \n \n Located in the Clifton Park Health Park, the practice offers convenient access to on-site laboratory services, medical imaging, women's health services, neurology, weight management, and numerous specialty services, allowing providers to deliver highly coordinated care within one integrated location. All CCP practices share a common electronic health record, promoting seamless communication and continuity of care. Responsibilities\n \n Provide comprehensive primary care services for adult patients age 18 and older \n Conduct annual wellness visits, preventive screenings, and routine health maintenance exams \n Evaluate, diagnose, and treat acute illnesses and chronic medical conditions \n Manage patients with hypertension, diabetes, hyperlipidemia, COPD, asthma, obesity, and other chronic diseases \n Order, interpret, and communicate laboratory, diagnostic, and imaging results \n Develop individualized treatment plans focused on prevention, wellness, and disease management \n Educate patients regarding medications, lifestyle modifications, and treatment goals \n Collaborate closely with the supervising physician and interdisciplinary care team to coordinate patient care \n Utilize telemedicine and patient portal technologies to enhance patient access and engagement \n Maintain accurate and timely documentation within the electronic medical record \n Qualifications Required\n \n Current New York State licensure as a Nurse Practitioner or Physician Assistant \n Graduate of an accredited NP or PA program \n DEA registration or eligibility \n Strong clinical assessment and decision-making skills \n Excellent communication and relationship-building abilities \n Preferred\n \n Previous experience in Internal Medicine, Primary Care, Family Medicine, or Urgent Care \n Experience managing chronic disease populations \n New graduates with a strong interest in adult primary care are encouraged to apply \n Schedule & Practice Highlights\n \n Full-time outpatient position \n Monday–Friday schedule \n Established adult patient population \n Telemedicine capabilities and integrated patient portal \n Access to on-site laboratory, imaging, and specialty services within the Clifton Park Health Park \n Shared electronic health record across CCP practices for coordinated patient care \n NCQA-recognized New York State Patient-Centered Medical Home (PCMH) \n Supportive physician-led organization focused on quality, patient-centered care \n \n \n Competitive benefits and compensation starting at $125,000 \n \n \n Required\n \n Current New York State licensure as a Nurse Practitioner or Physician Assistant \n Graduate of an accredited NP or PA program \n DEA registration or eligibility \n Strong clinical assessment and decision-making skills \n Excellent communication and relationship-building abilities \n \n
