Shift: Monday through Friday 8:30 am - 5:00 pm, On-call 1 Weekend/month

What You'll Do as an LPN Case Manager:As a vital part of our Home Care / Home Health team, your responsibilities will include:
- Provide nursing care to clients in their homes, facilities, wellness clinics, and always under the direction of an RN and according to the Nurse Practice Act.
- Delivering high-quality services to meet client needs, following current home health nursing professional standards and agency protocols.
- Identifying client needs for additional services and making appropriate referrals to managers or community resources.
- Maintaining current and accurate client records using laptop computers and traditional charting methods, in accordance with corporate policies and all federal, state, and local guidelines.
- Supervising and monitoring the performance of Homemakers and Personal Care Assistants in home care settings to ensure quality service delivery.
- Implementing physician-ordered plans of treatment.
- Assisting with orientation and field training for new employees.
- Participating in the department's on-call rotation as needed.
- A current, unencumbered Licensed Practical Nurse (LPN) license to practice in Pennsylvania.
- A valid Pennsylvania driver's license.
- Experience in home health or home care nursing is a plus, but all dedicated LPNs are encouraged to apply.
