Key Responsibilities\n

• Conduct detailed assessments of clients' psychosocial needs by evaluating emotional, mental, and social factors that affect their well-being.

• Collaborate with seniors and their families to create customized care plans tailored to their unique preferences and goals.

• Provide one-on-one counseling, emotional support, and resources to assist clients in managing life transitions, such as health challenges or loss.

• Actively advocate for clients’ rights, ensuring they have access to necessary community resources and services.

• Work in unison with an interdisciplinary team to deliver seamless and coordinated care that addresses all aspects of a senior's health.

• Facilitate conversations and mediate conflicts among family members regarding the client’s care to maintain positive relationships.

Qualifications\n

Compensation and Benefits\n

The compensation for this position ranges from $60,000 to $100,000 annually, commensurate with experience and qualifications.
