Infinite Medical P.C. is seeking a Licensed Practical Nurse (LPN) to serve as a Behavioral Health Care Manager within our Collaborative Care Model (CoCM) program. This is an in-office, non-face-to-face care management role supporting residents in skilled nursing and long-term care facilities.
