RN Case Manager Bellway Health and Rehabilitation Center – Selma, AL Full-Time | Skilled Nursing / Long-Term Care Bellway Health & Rehabilitation is seeking a compassionate and organized Registered Nurse (RN) Case Manager to join our skilled nursing team. This position plays an important role in coordinating resident care, supporting positive clinical outcomes, and ensuring effective communication among residents, families, providers, and the interdisciplinary care team. RN Case Manager Requirements Active Registered Nurse (RN)license in Alabama Experience in long-term care, skilled nursing, or rehab preferred Strong assessment, organization, and communication skills Ability to collaborate effectively with an interdisciplinary team Knowledge of care planning, discharge planning, and regulatory requirements Current CPR certification, or ability to obtain as required RN Case Manager Responsibilities Coordinate and monitor residents’ plans of care from admission through discharge Collaborate with nursing, therapy, social services, physicians, and other departments Communicate regularly with residents and families regarding care needs and progress Participate in interdisciplinary care plan meetings Assist with discharge planning and transitions of care Monitor changes in residents’ conditions and ensure appropriate follow-up Maintain accurate and timely clinical documentation Help ensure compliance with facility policies and state and federal regulations Advocate for residents and promote quality, resident-centered care Why Join Bellway Health and Rehabilitation Center We believe in supporting the people who care for our residents every day.
