We are committed to strengthening families and communities through compassionate care and evidence-based practices.We are excited to launch a new Milwaukee County team and are seeking Care Coordinators to support children with developmental, physical, or severe emotional disabilities through the Children's Long Term Support (CLTS) Waiver ProgramRole Overview As a CLTS Care Coordinator, you will serve as the primary point of contact for families, helping them access services that keep children safe and thriving in their home and community.
