Admissions Registered Nurse$4,500 sign on bonus, 4 x 10's, and every other weekend.The Admissions Registered Nurse conducts the initial admission process for patients referred for hospice services. The Admission nurse initiates the plan of care based on identified immediate needs, and communicates with the interdisciplinary team.QualificationsCurrent Florida license as RN, or current licensure from another state or compact license.Must be Certified Hospice and Palliative Registered Nurse (CHPN) within 12 months.Valid driver's license and automobile insurance in the State of Florida.Reliable transportation to meet visit schedules.Minimum two (2) years RN experience where physical assessment skills were utilized.Must be an RN graduate from a state approved school of nursing.Bachelor's degree in nursing preferred.Knowledge of Hospice Admissions criteria preferred.Current (BLS) CPR certification required.Prior experience in oncology, home health, Hospice or palliative care setting is preferred.Must be able to handle sensitive issues appropriately using sound judgment, maintain professional boundaries, and maintain confidentiality.Must be able to effectively plan, organize and prioritize work.Be self-directed and able to work independently in the field.Attendance is a condition of employment.ExpectationsResponsible for eligibility requirements for patients to access hospice services based on conditions of participation (COP).Provide program development strategies for Treasure Coast Hospice, in an organized, routine capacity that will enable the cohesion of team developments. Identifies the physical, social and emotional needs of the patient and family. Analyzes the assessment data in determining hospice appropriate diagnosis. Complies with state laws and regulations with respect to the hospice protocol and CMS guidelines for reimbursement.
