Responsibilities:

• Provide direct care to patients, including assistance with daily living activities, medication management, and vital sign monitoring

• Work closely with our skilled nursing team to develop and implement individualized care plans

• Maintain accurate and up-to-date records of patient care and progress

• Communicate effectively with patients, families, and healthcare professionals to ensure seamless coordination of care

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:
