As a **Home Health RN Field Nurse**, you willbuild from resources of the community to plan and direct services to meet the needs of patients and families within their homes and communities. **Some of your primary responsibilities will include:*** Completing an initial assessment of patient and family to determine home care needs. Providing a complete physical assessment and history of current and previous illness(es)* Supervising and directing the Licensed Practical Nurse (LPN) and/or the Home Health Aide in implementing the patient care plan* Developing patient education plans, as needed, and directing the LPN in implementing said plans* Creating and modifying the Home Health Aide care plan and supervising them via patient feedback and onsite interactions* Evaluating and revising plan of care as patient status and needs change* Using health assessment data to determine nursing diagnosis* Developing a plan of care, which establishes goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative action.* Administering medications and treatments as prescribed by the physician* Identifying discharge planning needs as part of the care plan development and implementing prior to discharge of the patient* Acting as the Case Manager when assigned by the Clinical Supervisor and assuming responsibility to coordinate patient care for assigned caseload* Preparing clinical notes as needed and communicating with the physician regarding patient’s needs**Benefits You Will Enjoy**In addition to competitive pay, full-time team member benefits include:* Paid Time Off* Insurance benefits (medical, dental, vision, life, and other voluntary group programs)* 401 (K) retirement savings plan with employer match* Tuition reimbursement for qualified career paths* Career advancement opportunities**Schedule**PRN As needed.**Education, Licensure & Certification Requirements*** Graduate of an accredited school of nursing.* Active RN licensure* One (1) to two (2) years of recent acute care experience in an institutional setting* Bachelor’s degree with one (1) year of home health care experience preferred* Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working orderlocations: Stonerise Home Health St. Clairsvilletime type: Full timeposted on: Posted 2 Days Ago #J-18808-Ljbffr
