Key Responsibilities:


- Collect and analyze relevant data to determine patient health needs.
- Develop comprehensive care plans that detail prescribed interventions for optimal health outcomes.
- Implement care plans and coordinate care delivery with a multidisciplinary team for enhanced patient safety and well-being.
- Evaluate patient progress and adapt care plans as necessary to ensure the best results.
- Collaborate with patients, families, and healthcare providers to create a supportive and healing environment.
- Provide valuable health information and learning opportunities to patients and families.
- Participate in discharge planning to ensure continuity of care.
- Delegate tasks appropriately and oversee the duties of healthcare team members.
- Assist with other job-related duties as assigned to advance team objectives.

Qualifications:


- Bachelor's Degree in Nursing (BSN) preferred.
- Acute care facility experience is a plus.
- Active Registered Nurse (RN) licensure in the state of practice.
- Current Basic Life Support (BLS) certification.
- Advanced Cardiovascular Life Support (ACLS) or Advanced Resuscitation certification required.
