As a PRN Home Health Physical Therapist (PT), you will play a crucial role in evaluating and treating patients in their homes, delivering top-quartile clinical outcomes and exceptional patient experiences. With a strong focus on whole-patient care, we offer a comprehensive benefits package, flexible scheduling, and opportunities for professional growth and development.Responsibilities:Determine physical therapy treatment plans in consultation with the care team and physicians through initial and ongoing assessments of the patient's condition.As a Home Health Physical Therapist (PT) provide and complete all proper documentation.Administer physical therapy treatments as ordered by a physician, directing and overseeing treatment given by physical therapy assistants.Evaluate the effects of physical therapy treatments by observing, noting, and evaluating patients' progress, and recommending adjustments and modifications.Complete discharge planning by consulting with the care team and physicians and assure any appropriate continuation of the therapeutic plan post-discharge.Participate in weekly case conferences and effectively communicate about patient care across disciplines involved.Qualifications:Minimum of 1 year of physical therapy experience, with a preference for 2+ years in an acute or post-acute care setting.Active state licensure as a Physical Therapist (PT) is required.Proof of current CPR certification.Valid driver's license.Why Join LiveWell Partners?At LiveWell Partners, we prioritize the well-being of our team members and strive to provide a fulfilling career where every individual can positively impact our patients' lives.
