Pay Rate: $ 1900 To $ 2161 per week

About the Role

We are seeking an experienced Physical Therapist (PT) Home Health for a 13-week contract position covering Fremont and surrounding East Bay communities. The Physical Therapist will provide home-based evaluations, treatments, reassessments, and rehabilitation services while traveling between patient locations.

Responsibilities

• Perform comprehensive home health physical therapy evaluations and develop individualized plans of care based on mobility, function, safety, and rehabilitation goals.

• Provide skilled physical therapy treatments, reassessments, OASIS-related visits, start-of-care, recertification, and discharge services as assigned.

• Educate patients and caregivers regarding therapeutic exercises, mobility, fall prevention, assistive devices, safety, and home exercise programs.

• Complete timely and accurate clinical documentation and coordinate care with physicians and interdisciplinary healthcare professionals.

• Travel throughout the assigned service territory and surrounding areas while meeting required weekly visit and productivity expectations.

Requirements

• Active California Physical Therapist (PT) License Required.

• Minimum 1 Year of Physical Therapy Experience Required; Home Health Experience Strongly Preferred.

• Current BLS/BCLS Certification Required.

• Valid Driver's License, Current Auto Insurance, and Vehicle Registration Required.

• Must Meet Required Health and Compliance Credentials, Including Current TB, MMR, Varicella, Hepatitis B, Tdap, Flu, Drug Screen, and Physical Requirements.

The Home Health Physical Therapist will provide independent, patient-centered rehabilitation services designed to improve mobility, function, safety, and independence in the home setting.
