Physical Therapist Assistant (PTA) Opportunity

Fifth Star Care Management | In-House Therapy Team

Fifth Star Care Management is seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our IN-HOUSE therapy team! New Grads Welcome!

We are looking for a PTA who is passionate about providing high-quality, patient-centered care and helping residents improve mobility, strength, balance, physical function, and overall quality of life.

• Implement physical therapy treatment plans developed by the Physical Therapist in accordance with physician orders and facility protocols

• Provide therapeutic interventions including exercises, gait training, balance activities, functional mobility training, and use of assistive devices

• Monitor patient progress and report any changes or concerns to the supervising Physical Therapist

• Assist residents with mobility and physical functioning to support independence in daily living activities

• Educate residents, caregivers, and staff on safe mobility techniques and proper use of adaptive equipment

• Maintain accurate and timely documentation of treatments, progress notes, and other required records

• Ensure a safe, clean, and supportive environment for all residents

• Collaborate with interdisciplinary team members, including nurses, occupational therapists, and speech-language pathologists, to support resident care plans

• Participate in care planning and communicate relevant patient observations to the therapy team

• Adhere to all applicable federal, state, facility, and professional regulations and policies

• Associate degree from an accredited Physical Therapist Assistant program

• Current state licensure or certification as a Physical Therapist Assistant

• Prior experience in a long-term care, skilled nursing, or geriatric setting preferred

• Strong communication, organizational, and interpersonal skills

• Compassionate, dependable, and patient-focused approach to care

• Ability to work effectively as part of a team and independently with minimal supervision
