Physical Therapist (PT) - Skilled Nursing Facility

Physical Therapist

Schedule: 4x10-hour shifts, Friday to Monday.

Assignment Details: Contract length: 13 weeks. Guaranteed hours: Full-time.

Requirements:

Experience as a Physical Therapist.
Comfortable with weekend responsibilities.
Outpatient experience required.
Familiarity with EMR systems, preferably EPIC.

Responsibilities:

Provide outpatient physical therapy services.
Evaluate and develop treatment plans for patients.
Implement therapeutic interventions and adjust as necessary.
Document patient progress and updates in the EMR system.
Collaborate with healthcare team members to optimize patient care.
Educate patients on exercises and home care programs.

Compliance / Notes: Candidates must meet compliance requirements 2 weeks prior to the start date.
