The position to which you are applying is represented by a collective bargaining unit, Massachusetts Nursing Association. At Mass General Brigham Home Care, Physical Therapists deliver skilled, personalized care to patients recovering from illness, injury, surgery, or joint replacement, as well as those managing chronic, cardiac, respiratory, or neurological conditions. In the home, you see the full picture—tailoring treatment to each person’s real environment while helping patients restore mobility, reduce pain, prevent falls, and regain confidence and independence. What You’ll Do •Complete comprehensive physical therapy evaluations in patients’ homes. •Develop and adjust individualized care plans with measurable, patient-centered goals. •Provide skilled interventions to improve strength, balance, mobility, endurance, and function. •Educate patients and caregivers on exercises, fall prevention, and safe mobility strategies. •Recommend and coordinate adaptive equipment and additional services. •Collaborate closely with the interdisciplinary team and referring providers. •Document care accurately and efficiently using mobile technology.
