Job Title: Physical Therapist

Experience: Minimum 1 year of adult home health experience


License: Must have a valid license


Certifications: CPR, BLS


Must-Have: Recent home health experience


Description:
Home visits required for therapy services.
Periodic office meetings are expected.
Must have reliable transportation and a current driver's license.
Agency-required liability insurance is necessary.
Average number of patients per day is 5-6.
Contractor will be assigned between 5-10 patients daily.
This equates to roughly 6-8 points.
Weekly productivity expectations are a minimum of 30 points.
Each type of visit and activity has a different point value.
Types of cases include start of care and follow-up visits.
Must be willing to travel up to 75 miles from the home clinic.
Mileage is reimbursed at the IRS standard rate.
Commute to and from the first and last patient is not billable.
Local candidates are accepted at the same rate.
Must hold a valid license prior to start.
Must be proficient in specific electronic medical record systems.
Documentation must be completed within 24 hours of the appointment.
Declining patients may result in cancellation of contracts.
No guaranteed hours with this contract.
Clinician must be open to on-call rotations.
Must accept every patient on their schedule regardless of distance.
Potential for floating to another location to pick up additional hours.
All overtime must be pre-approved.
