Location 401 Ridge Street,Saint George, SC, 29477,United States Base Pay $24.00 - $28.00 / Hour Employee Type FT Non-Exempt Required Degree 2 Year Degree Manage Others No Description What You'll Do As an LPN, you will work closely with providers, nurses, and other members of the care team to provide comprehensive clinical support, including: Assess and monitor patients' conditions and conduct health risk assessments. Prepare patients for examinations, treatments, and diagnostic procedures. Administer medications, treatments, and immunizations as ordered by providers. Reconcile medications and maintain accurate medication lists in the electronic health record. Provide education to patients regarding medications, treatments, health maintenance, disease prevention, and healthy lifestyle choices. Obtain specimens and perform appropriate in-house laboratory testing. Coordinate and track laboratory, radiology, and other diagnostic results and follow up with patients as directed. Assist with identifying and addressing gaps in care and support patient panel management. Coordinate and track referrals, including referrals to community resources. Maintain accurate, timely, and complete documentation in the electronic health record. Participate in daily team huddles and pre-visit planning. Maintain clinical supplies and ensure appropriate inventory for the care team. Maintain quality control of in-house testing equipment and monitor/document refrigerator temperatures. Sterilize equipment according to established protocols. Follow all infection prevention, safety, and biohazardous waste procedures. Provide professional, compassionate, and respectful service to patients, families, visitors, and coworkers. Support the overall goals of the clinical team and perform other duties as assigned. Requirements Required Qualifications Graduate of an accredited Practical Nursing program. Current, unrestricted South Carolina LPN license . Current CPR certification . Strong communication and interpersonal skills. Ability to maintain patient confidentiality and exercise sound professional judgment. Basic computer skills and ability to use electronic health records. Ability to work effectively in a fast-paced, team-oriented healthcare environment. Preferred Qualifications Two years of LPN experience in a healthcare setting. Experience in primary care, ambulatory care, community health, or a federally qualified health center (FQHC). Experience with electronic medical records/EHR systems. Experience with care coordination, referrals, patient education, or population health activities. What Makes You a Great Fit?
