Case Manager (RN or LPN) – Full‑Time The Case Manager’s primary function is to provide patients, their families, and/or significant others with special services to facilitate adjustment of their continuity of care after discharge. The Case Manager oversees effective coordination of admission and discharge planning, case communications, interdisciplinary team conferences, insurance relations and assurance through managed care systems, and verbal and written reporting. Assures compliance with regulations regarding patient rights.
