Role Description The Case Manager is a full-time, on-site role based in Plainfield, IL, responsible for coordinating and overseeing comprehensive care for home healthcare clients. This position involves assessing client needs, developing individualized care plans, and collaborating with nursing staff, home health aides, maternal support specialists, and other service providers. The Case Manager monitors progress, adjusts care plans as needed, and ensures services are delivered in alignment with clinical guidance and client preferences. Day-to-day tasks include conducting home visits, documenting assessments and interventions, facilitating communication among families and interdisciplinary teams, and helping clients access community resources and support services. The role also includes maintaining regulatory and documentation compliance while promoting high standards of quality, safety, and client satisfaction. Qualifications Clinical care coordination skills, including experience in case management, care planning, and interdisciplinary collaboration. Strong assessment, documentation, and organizational skills, with the ability to manage multiple cases and priorities effectively. Excellent communication and relationship-building skills to work with clients, families, and healthcare professionals in diverse home settings. Knowledge of home healthcare practices, community resources, and relevant state and federal regulations. Licensed RN, LPN, social worker, or other relevant clinical/healthcare credential preferred, with prior experience in home health or related settings. Ability to travel locally for home visits, maintain confidentiality, and use electronic health records or case management software.
