Case Management Director

Community hospital part of a system looking to bring on Case Management Director! Bonus Incentive Program, Sign On Bonus and Full Relocation!
- Overall responsibility for managing and coordinating department activities.
- Ensures staff compliance with organizational policies and external regulatory agencies and takes leadership responsibility to coordinate the integration of the department's patient care and discharge planning processes.
- Oversight into all aspects of daily hospital case management operations and is accountable for achieving established outcomes.
- Reports to Division Director of CM.
- Overseeing 13 employees (1 lead CM, 1 CMA, 2 SW, 9 RN CM - 4 are PRN).
Qualifications:
- Bachelor’s Degree
- Master's degree in Nursing, Health Administration, or Business Administration, preferred
- (RN) Registered Nurse, or (LCSW) Clinical Social Worker, (RN) Registered Nurse preferred
- 3 years’ experience in overall acute care Hospital Case Management, preferred
- 2+ years’ experience in case management leadership in an acute care setting MANDATORY
