32 hours M-F, No Weekend Rotation

Would share coverage between 2 rehab sites, 19 miles apart.

2 days at one, 3 days at the other

Experience/Qualifications

• OT license and state certification with current CPR/First Aid

• Provide on-site OT assessments and individualized treatment plans

• Document progress notes and maintain compliant client records

• Adhere to ethics, privacy laws, and workplace safety guidelines

• Set measurable goals aligned with patient needs and program targets

• Collaborate with multidisciplinary team to optimize outcomes

• Pipeline-driven caseload planning and progress tracking

• Educate patients and caregivers on therapy plans, home exercises, and safety

• Participate in performance metrics reporting and productivity tracking

• Maintain ongoing professional development and certifications

• Demonstrate reliability by meeting deadlines and individual targets

• Participate in on-site program marketing and referral outreach
