ASA VII - Case Management Assistant

Department/Unit: Care Management/Social Work Work Shift: ED DIFF_EVENING (United States of America) Salary Range: $46,215.00 - $60,080.00 Responsibilities Assist CM/SW staff with setting up patients’ discharge needs, primarily discharging from the ED (evening position). Obtain Durable Medical Equipment (DME) options, including rolling walkers, commodes, wheelchairs, quad canes, and hospital beds. Make basic transportation arrangements (MAS, rehab, back transfer, cabs) once the discharge planner has documented the need. Maintain patient confidentiality of all communications and documents as required by hospital policy. Assist with fax and print attachments, and enter information into Allscripts. Round frequently on assigned units. Keep Case Managers and Social Workers apprised of the status of discharge plans, document activities and communications, and maintain adequate reporting information in Allscripts. Maintain daily documentation in Allscripts of all activities performed using interventions and enter data in a timely fashion to facilitate discharge plans. Maintain skills needed for performance of duties and utilize available educational opportunities for skill improvement. Attend required in‑house in‑services. Project a positive image of the department and hospital. Qualifications & Ideal Characteristics HS diploma or equivalent and 3-5 years of relevant experience. Proficient with computer/software applications. Strong written and verbal communication skills and organizational skills. Ability to multitask, prioritize, and work effectively in a fast‑paced environment. Physical Demands Regularly required to stand, walk, use hands to probe, handle, or feel objects, tools, or controls, reach with hands and arms, and speak and hear. Occasionally required to sit, stoop, kneel, or crouch. Must regularly lift and/or move up to 50 pounds. Specific vision abilities required: close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. Work Environment The work environment may involve exposure to infectious materials and chemicals. Noise level is usually moderate. This position requires a physical presence on‑site for the majority of the time, as remote work is not viable.
