Home Health PT Physical Therapist Full Time

Position Overview Seeking a Physical Therapist (PT) to join a home health care team, enabling patients to recover at home. Full-time weekly visits are required for Osceola County . Key Responsibilities Perform initial and ongoing assessments of mobility, strength, and function. Administer, evaluate, and interpret tests to set functional goals. Provide written evaluations and care plans outlining needs, goals, and progress. Deliver physical therapy services per physician orders, documenting performance and adjusting treatment plans as needed. Manage discharge planning, including patient education on home exercise routines. Coordinate with healthcare team members, including physicians and caregivers. Ensure accurate documentation to prevent denials and maintain compliance with policies and regulations. Supervise personnel such as PTAs and HHAs where applicable. Complete OASIS assessments as permitted by regulation. Qualifications Degree in Physical Therapy from an accredited program. Current, unrestricted state license. Valid CPR, driver's license, and proof of auto insurance. Preferred Skills Experience in home health therapy and OASIS documentation.
