Position Overview

Caremed Inc. is seeking an experienced Patient Care Coordinator (Home Care Scheduler) to join our home health operations team. This critical role is responsible for end-to-end patient care coordination, caregiver scheduling, Electronic Visit Verification (EVV) compliance, and daily client service delivery.

Core Responsibilities & Essential Duties

• Caregiver & Patient Scheduling: Oversee daily roster management, matching home health aides and caregivers with client care plans across Central Virginia.

• Shift & Coverage Management: Manage open shifts, call-outs, schedule changes, urgent cancellations, and immediate reassignments to ensure uninterrupted patient care.

• Electronic Visit Verification (EVV): Perform real-time EVV tracking, visit verification, attendance reconciliation, and resolve EVV exceptions for billing compliance.

• On-Call Support: Participate in a rotating on-call schedule for weekend and after-hours emergency dispatching and care coordination.

• Interdisciplinary Communication: Serve as the primary liaison between patients, family members, field caregivers, RN Supervisors, and administrative management.

• Documentation & Compliance: Maintain precise, HIPAA-compliant patient files, authorization records, and visit logs within care management software platforms.

Preferred Qualifications & Core Competencies

• Education: High School Diploma or GED required; Associates or Bachelors degree in Healthcare Administration, Business, or related field is a plus.

• Experience: 2+ years of experience in healthcare scheduling, medical office administration, home health coordination, or patient services.

• Technical Skills:

  • Proficiency in Electronic Visit Verification (EVV) software and Electronic Health Records (EHR).

  • Strong working knowledge of Microsoft Office Suite (Word, Excel, Outlook, Teams).

  • Experience with Medicaid authorizations, Plans of Care (POC), and home care compliance standards.

• Soft Skills: Outstanding phone etiquette, conflict resolution, crisis management, multitasking, and organizational skills in a fast-paced environment.
