Job Title: Hospice Registered Nurse Profession: RN Specialty: Hospice RN Duration: 13 weeks Shift: Days: Sunday - Thursday Hours per Shift: 10am - 7pm or 11am - 8pm (Schedule can be flexible based on patient needs) Experience: Minimum of 1 year of Hospice experience License: Active RN License Certifications: BLS Certification Must-Have: One year of Hospice experience. Description: The Hospice Nurse admits patients to service. They assess the patient's and family's physical, psychosocial, environmental, safety, and developmental needs. They develop an individualized plan of care based on their assessment. They document their observations and interventions related to the patient's environment. This role coordinates care with a multidisciplinary team. They participate in the hospice program's quality assessment performance improvement program. They conduct follow-up visits as needed. Territory details include various counties for patient coverage. Holiday coverage is required.
