1. Perform comprehensive medication reviews (cmrs) and targeted medication reviews with patients.
2. Identify medication-related problems, drug interactions, adherence barriers, and cost-saving opportunities.
3. Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
4. Educate patients and caregivers on safe, effective medication use.
5. Maintain compliance with cms mtm program requirements and quality metrics.

what you'll bring

• education: pharmd or bachelor of pharmacy.
• licensure: active u.s. pharmacist license.
• experience: ambulatory, hospital, community, or prior mtm experience is a plus. All licensed pharmacists, including new graduates, are encouraged to apply.
• skills: excellent patient counseling, independent work style, and clear documentation.
