The Occupational Therapist (OT) provides skilled therapy services to patients in their homes, focusing on improving independence in daily living activities, safety, and overall functional ability. The OT evaluates patient needs, develops individualized care plans, and collaborates with the interdisciplinary team to promote optimal outcomes.

Compensation:

$55 - $75 hourly

Responsibilities:

• Perform comprehensive initial evaluations to assess patient functional status, including ADLs, mobility, strength, cognition, and home safety

• Develop and implement individualized treatment plans based on physician orders and patient goals

• Provide skilled occupational therapy interventions to improve independence in activities of daily living (ADLs) and instrumental ADLs

• Conduct home safety assessments and recommend adaptive equipment and environmental modifications

• Educate patients and caregivers on disease management, safety, and use of assistive devices

• Complete all required documentation, including OASIS assessments , progress notes, and discharge summaries, in a timely and accurate manner

• Communicate regularly with physicians, nurses, and other team members regarding patient status and plan of care

• Participate in care coordination and case conferences

• Ensure compliance with all Medicare, state, and agency regulations

• Maintain productivity standards while delivering high-quality patient care

Qualifications:

• Graduate of an accredited Occupational Therapy program
• Current Occupational Therapist license in the State of Michigan
• Minimum 1 year of clinical experience (home health experience preferred)
• Valid driver’s license, reliable transportation, and auto insurance
• Current CPR certification
• Strong clinical, communication, and documentation skills
• Experience with OASIS documentation preferred
