Nephrology Physician

We are seeking an experienced Medical Director with a background in Cardiology, Nephrology, or Primary Care to lead an integrated clinical care model focused on primary care coordination and specialty collaboration. Key Responsibilities Include Providing clinical and administrative leadership to a multidisciplinary team including nurse practitioners, physician assistants, and nurse case managers. Driving population health outcomes by reducing hospitalizations, emergency visits, and readmissions. Supporting primary care integration, clinical quality, and developing a strong referral network. Overseeing provider workflows, implementing evidence-based protocols, and facilitating team feedback and case reviews. Promoting equitable care through outreach to underserved communities and strategic growth planning. Serving as a clinical liaison with partners, payers, and health system leaders. Qualifications And Skills MD or DO with board certification in relevant specialties. Active or eligible Georgia medical license. At least 2 years of clinical leadership or administrative experience. Knowledge of managed care, quality metrics, and population health principles. Strong communication, collaboration, and problem-solving skills.
