PCA II: Care & Coordination Specialist

AltaMed Health Services Corp. seeks a PCA II to perform and assist with personal care ADLs/IADLs for participants in the PACE program across center, home, or virtual settings. The PCA II may assume PCA I duties as needed and assists with gathering information for care coordination and tracking participant well-being.
