Active Connecticut Physical Therapist license required to start the assignment. BLS certification from American Heart Association or American Red Cross may be required. Experience working in a Skilled Nursing Facility or long-term care setting preferred. Demonstrated proficiency with patient mobility training, fall prevention programs, and documentation in an electronic medical record preferred. Ability to manage a caseload in a fast-paced rehabilitation environment and collaborate with interdisciplinary teams. Assignment Details Facility Type: Skilled Nursing Facility. Shift Name: Days.
