Malpractice Must be included in rates.

Orientation $257.50-262.50 - half the hourly/callback rate, kicks in after 8 hours. "Per hour of orientation or training beyond 8 hours"

Hourly $515-525 - "Per hour, not OT"

Holiday $772.50-787.50 - Must be 1.5x the hourly rate. "Per holiday hour, no OT"

Schedule/Availability Requirements:

Daily/weekly coverage needed Monday - Friday; 7a - 3p or 7a - 5p

No Night Call or Weekend Call Needed

Duties: General anesthesia

Required Procedures: Ortho, Spine, Gynecology, Urology, Interventional Radiology, Neurosurgical: Spine and Intracranial, General Surgery, Robotic, ENT: Head and Neck, Pediatric, Vascular: Open and interventional, Advanced GI procedures

EMR: Epic

Support Staff: n/a

Reason for Coverage: MDA leaving group

Credentialing Information:

Timeframe: 90 days - need to fully credential @ CHI Health Good Samaritan & with CHI Health Partners

Requirements:

Board Certification Board Certified [REQUIRED]

State License Required or IMLC [HIGHLY PREFERRED], will accept providers without active license in hand. must obtain prior to start date.

Malpractice Clean Preferred [HIGHLY PREFERRED]

Availability at time of name clear [REQUIRED]

The Self-Query NPDB report must be dated within 30 days of the presentation date.
