Overview Sign on Bonus Available! Piedmont Eastside The PEM Care Management department is made up of a diverse group of individuals. Each come with their own unique backgrounds, skills, and experiences. The department is comprised of Social workers, RNs, and CMAs. Together we have built a team that can draw on each other's strengths and supplement each other's weaknesses. We expand each other's knowledge and skills through collaboration and coaching, and provide opportunities for growth within the department and organization. We work together as a team every day to expedite the most appropriate and timely discharges for our patients. We work collaboratively with other members of the interdisciplinary team, as well as post-acute providers to ensure continuity of care. We do this and so much more everyday as a team! Responsibilities The Care Manager is responsible for care coordination, progression of care, and proactive discharge planning and is accountable for expediting the timely and safe discharge for all patients in their case load. Qualifications Education Graduate of Nursing Program Required Masters degree Preferred Work Experience 2 years of experience in an acute or post-acute setting Required Experience using InterQual or Milliman Preferred Licenses and Certifications RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Required Certification in Case or Care Management Preferred #J-18808-Ljbffr
