MedCap Health is seeking a full-time Assisted Living APP to help reshape the future of post-acute care. You'll spend your days managing patients with acute and chronic diseases, collaborating directly with the attending physician to provide the highest quality of care. Your expertise is crucial in effectively managing patients with debilitating chronic diseases, improving their quality of life. You are a natural collaborator, communicating and working seamlessly with the Director of Nursing, Social Workers, and other team members to ensure a cohesive and comprehensive approach to care. This dynamic role keeps you engaged and connected as you make a tangible difference in the lives of those you serve.OUR IDEAL PRACTITIONERA board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)1+ years of experience in post-acute/SNF/LTCPrior post-acute, long-term, and/or skilled nursing facility experienceWHO WE ARE:As a physician-led and focused organization, MedCap Health is helping reshape the clinical landscape of post-acute care.
