Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc. must be in a manner that is courteous, respectful, cooperative and professional. This behavior should promote an atmosphere of teamwork and positive relations.**ESSENTIAL FUNCTIONS**• Provides evaluation and management for assigned patients.• Obtains history, physical exam and formulates assessment and plan of care (POC) for patients assigned to APRN• Triages patient care problems.• Teaches patient and caregivers (including facility staff) regarding disease process, death and dying, comfort measures, medication use and side effects, etc.• Develops, maintains, coordinates and adapts POC as needed.• Provides psychosocial support to patients and families.• Keeps timely and accurate medical records## Qualifications* Graduation from an approved school of professional nursing as determined by the Board of Nurse Examiners with degree as Nurse Practitioner.* Unrestricted license as a Registered Nurse by the Texas Board of Nurse Examiners.* Valid Texas driver’s license, auto liability insurance and safe driving record.* CPR certification.**PREFERRED:*** 1 year or more nurse practitioner experience* Prior hospice or palliative care experience #J-18808-Ljbffr
