Luxe Home Health is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team. This is a per diem position offering flexibility and the opportunity to make a difference in the lives of our patients.

As an LPN at Luxe Home Health, you will play an essential role in providing high-quality care to patients in their homes, working under the direction of a registered nurse. You will work with a supportive team to ensure patients' needs are met while maintaining the highest standards of care. Your responsibilities will include administering medications and treatments as prescribed by physicians, monitoring and assessing patients' conditions while reporting any changes to the clinical team, and providing education to patients and their families regarding care plans and health management. You will collaborate with other healthcare professionals to ensure comprehensive care and maintain accurate patient records and documentation according to policies and regulations.

Requirements for the LPN:

• Current, valid LPN license in the state of Indiana.
• Minimum of 2 years of experience as an LPN in a home health setting.
• Ability to work independently and manage time efficiently.
• Compassionate and patient-centered approach to care.
• Valid IN driver’s license, reliable transportation, and the ability to travel within the service area.
