UnitedHealth Group's Patient Care Hospice is hiring a Part Time Registered Nurse Case Manager to deliver the Plan of Care for hospice patients. You will collaborate with the care team to promote physical, psychosocial, emotional, and spiritual well-being. You will conduct initial evaluations within 48 hours, develop and coordinate the plan of care, and document progress while guiding patients and families through care options.
