Care Coordinator for Adults with Disabilities

The Care Coordinator – Adults with Disabilities is responsible for coordinating and managing nonmedical home- and communitybased services for adult clients with intellectual and developmental disabilities (IDD) and/or physical disabilities across Cuyahoga and Lorain Counties . This role serves as the primary point of contact for clients, families/guardians, Direct Support Professionals (DSPs)/Caregivers, and community partners; develops and monitors individualized service plans; schedules staff; ensures compliance with agency, payer, and regulatory requirements; and drives excellent outcomes and client satisfaction. Key Responsibilities Service Coordination & Care Planning Complete intake, assessment, and personcentered service planning in collaboration with the client, guardian, and interdisciplinary team. Develop, implement, and update Individual Service Plans (ISPs)/Care Plans with measurable goals, risk mitigation, and support strategies. Coordinate transitions of care (e.g., hospital to home), ensuring continuity of services and medication reconciliation support within scope. Scheduling & Staffing Build and maintain weekly schedules for DSPs/caregivers; manage calloffs, lastminute needs, and coverage across both counties. Match staff to clients based on skills, preferences, geography, and authorization requirements. Track staff attendance and timekeeping; elevate issues promptly. Client/Family Communication & Advocacy Serve as primary contact for assigned caseload; respond to inquiries, concerns, and incident reports in a timely, compassionate manner. Educate clients and families on services, resources, rights, and responsibilities. Advocate for leastrestrictive, communityintegrated supports and selfdetermination. Quality, Compliance & Documentation Maintain accurate, timely documentation in the EMR/scheduling platform (e.g., service notes, ISPs, incident reports, consents). Ensure services align with payer authorizations (Medicaid HCBS waivers, county boards, managed care) and agency policies. Conduct home visits and supervisory checkins; complete satisfaction calls and annual/periodic reviews. Support audits and quality improvement initiatives; follow HIPAA and confidentiality standards. Coordination with Community Partners Collaborate with county boards of DD, managed care organizations, DODD/Area Agency on Aging, hospitals, clinics, and community providers. Participate in interdisciplinary team meetings, ISP reviews, and case conferences. Facilitate access to ancillary services (transportation, respite, therapies, employment supports) as appropriate. Risk Management & Safety Identify and address health and safety risks; follow incident management and reporting protocols. Promote infection control and safe inhome practices; provide caregiver coaching within scope. Qualifications Required Associate’s degree in Social Work, Human Services, Nursing, Psychology, or related field or equivalent experience in care coordination/case management. 1–3 years of experience supporting adults with disabilities (IDD and/or physical disabilities) in home- or communitybased settings. Working knowledge of Medicaid/HCBS waivers and local systems of care in Ohio (Cuyahoga & Lorain). Strong organizational and scheduling skills; ability to manage a dynamic caseload. Excellent interpersonal communication, cultural humility, and problemsolving abilities. Valid Ohio driver’s license, reliable insured vehicle, and ability to travel daily within both counties. Proficiency with EMR/scheduling software and Microsoft 365/Google Workspace. Preferred Bachelor’s degree in Social Work (BSW), Psychology, Public Health, Nursing (LPN/RN), or related. Familiarity with Ohio DODD rules, county board processes, and personcentered planning. Experience supervising DSPs/caregivers and/or staffing coordination. Schedule & Work Environment Fulltime, generally Monday–Friday with oncall rotation for urgent scheduling needs. Hybrid field/office role with frequent inhome visits across Cuyahoga and Lorain Counties. Occasional evenings/weekends to meet client needs. Physical & Other Requirements Ability to lift up to 25 lbs occasionally, conduct home visits (stairs, varied environments), and sit/stand for extended periods. Satisfactory background checks, driving record, and drug screening per policy. Proof of eligibility to work in the U.S. and required vaccinations per agency policy. Key Performance Indicators (KPIs) Timely completion of intakes, ISPs, and documentation (e.g., within 24–72 hours). Visit and supervisory checkin compliance rates. Schedule fillrate and ontime coverage. Client/family satisfaction scores and retention. Authorization utilization accuracy and audit readiness.
