Physical Therapist

We are seeking three full-time or part-time positions, servicing Katy, Fulshear, Weimar, Sealy, Bellville, Brenham TX, and surrounding communities. 1st Texas Home Health is a licensed home health agency delivering skilled nursing, physical, occupational, and speech therapy services in patients’ homes. Role Description The Physical Therapist will provide comprehensive physical therapy services to home health patients, including assessment, treatment planning, and ongoing rehabilitation interventions. Responsibilities include performing evaluations, developing and updating individualized care plans, and delivering therapeutic exercises and functional mobility training to improve safety and independence at home. The role involves close coordination with physicians, nurses, and other therapists, as well as educating patients and caregivers on home exercise programs, assistive devices, and fall prevention. The Physical Therapist will document all care in accordance with regulatory and agency standards and participate in case conferences to support continuity of care. Qualifications Strong foundation in Physical Therapy and Rehabilitation, with the ability to design and implement evidence‑based treatment plans. Knowledge of Medicine and clinical practices relevant to home health, including understanding of common diagnoses and comorbidities. Demonstrated skills in Patient Care and Home Care, focusing on safety, functional independence, and patient‑centered outcomes. Current license as a Physical Therapist in the state of Texas. Experience in home health or community‑based care preferred. Ability to work independently in the field, manage a caseload, and travel locally to patient homes. Effective communication, documentation, and interpersonal skills to collaborate with patients, caregivers, and interdisciplinary teams.
