We are seeking an exceptional, highly experienced Physical Therapist to deliver elite, patient-centered care in a home-based setting. This role is designed for a clinician who values autonomy, clinical excellence, and meaningful one-on-one patient relationships. You will play a critical role in helping patients regain independence, improve function, and enhance their overall quality of life—all within the comfort of their homes.
We offer premium per diem or visit-based compensation and mileage reimbursement
Key Responsibilities:
- Perform comprehensive in-home evaluations and design advanced, individualized treatment plans
- Deliver high-quality, evidence-based physical therapy services
- Continuously assess and optimize patient progress with clinical precision
- Provide expert education to patients and families to support long-term success
- Maintain meticulous, compliant documentation
- Collaborate with a multidisciplinary care team to ensure seamless patient outcomes
- Uphold the highest standards of professionalism, safety, and patient care
Qualifications:
- Licensed Physical Therapist (State of Michigan) is required
- Proven experience in home health or a comparable independent-care setting preferred
- Reliable transportation and willingness to travel locally
- Advanced experience with geriatric and medically complex patients
- Strong understanding of home health regulations and best practices
- Background in functional mobility, fall prevention, and post-acute care
- Demonstrated clinical excellence and strong critical thinking skills
- Ability to thrive independently while managing a dynamic schedule
