RN Case Manager

Overview RN Case Manager / Nurse III position with Health & Human Services Commission. Location: El Paso State Supported Living Center. Not eligible for telework. Salary: $7,532.75–$8,886.16 per month. Shift: Day; Additional shift: Days (First). Brief Job Description The RN Case Manager performs complex nursing work providing assessment, care and integrated treatment of individuals residing at the state-supported living center. Works as part of the interdisciplinary team to ensure high-quality, holistic care. Conducts physical assessments, develops health care plans, oversees nursing interventions, and evaluates outcomes. Maintains compliance with nursing best practices, documents care, and communicates with other disciplines to ensure continuity of care. May mentor and coach new RN Case Managers and participate in committees and quality assurance activities. Provides treatment to HHS employees injured on the job as needed. Works under the supervision of a Registered Nurse manager and uses initiative and independent judgment. Essential Job Functions (EJFs) Attends work on a regular basis and may be required to work a specific shift schedule, rotating schedule, extended shifts, or overtime in accordance with agency policy. May serve in an on-call status on a rotating schedule. Monitors health status of individuals, conducts rounds, observes and communicates findings, ensures nursing interventions are implemented, analyzes outcomes, and documents issues per protocols. Provides hands-on nursing care as needed and guides LVNs and direct care staff in implementing health care plans. Collaborates with the IDT to solve problems related to care and health care plans. Updates records and communicates with other disciplines. Participates in required meetings, quality assurance activities, and professional development. Attends on-campus and off-campus medical appointments as needed to coordinate care. Maintains safety and infection control practices and complies with licensure and continuing education requirements. Maintains professional relationships and mentors new staff. Assists with disaster response and continuity of operations when required. Other duties as assigned related to nursing policy, procedure refinement, and service delivery. Knowledge, Skills and Abilities (KSAs) Knowledge of nursing techniques, standards, and procedures; health care laws and regulations; medical diagnoses and procedures; treatment patterns; community health and nursing care principles; program regulations; and utilization review concepts. Skill in patient care and in using computer/software and electronic health records. Knowledge of state/federal laws and ICF-IID rules related to State Supported Living Centers or ability to quickly gain it. Ability to interact with individuals with IDD, their families, and staff; interpret policies and regulations; and recognize patterns of medical necessity and treatment. Ability to perform comprehensive physical assessments, develop care plans, and evaluate outcomes; strong time management and prioritization skills. Excellent verbal and written communication; ability to work independently with all levels of staff. Current CPR certification and ability to perform CPR; knowledge of emergency procedures and equipment. Ability to maintain licensure and comply with safety and infection control practices; ability to interpret regulations and communicate them in writing and orally. Registrations, Licensure Requirements or Certifications Licensed to practice as a registered nurse in Texas or in a state with reciprocity through the Nurse Licensure Compact. Qualifications and Experience At least 1 year of experience as a practicing registered nurse required. Experience in assessment and documentation of health status required. 2+ years in a health agency, hospital, or health-care facility preferred. Bachelor’s Degree preferred. Prior experience with individuals with IDD preferred. Additional Information Applicants must pass fingerprint background check, pre-employment drug screen, and required registry checks.
