We are hiring a Physical Therapist for a 13-week contract in Napa, CA. This is a home health role covering assigned territories, including Vacaville, Fairfield, Rio Vista, Suisun City, Dixon, Vallejo, Benicia, Napa, American Canyon, and Yountville.

About the Role
We are hiring a Physical Therapist for a 13-week contract in Napa, CA. This is a home health role covering assigned territories, including Vacaville, Fairfield, Rio Vista, Suisun City, Dixon, Vallejo, Benicia, Napa, American Canyon, and Yountville.
Responsibilities
- Provide physical therapy services in a home health setting
- Complete evaluations, routine visits, reassessments, recertifications, and discharges
- Develop and follow individualized patient treatment plans
- Support mobility, strength, balance, safety, and functional independence
- Travel within assigned territory and outside the area as needed
- Complete patient documentation accurately and timely
- Communicate with patients, physicians, and care team members
- Meet weekly productivity and visit expectations
- Active California Physical Therapist license required
- Minimum 1 year of Physical Therapy experience required
- Home health experience preferred
- BLS/BCLS certification required
- Valid driver's license, auto insurance, and auto registration required
- Must be willing to travel within assigned territory
- Weekend coverage may be required
- Must be willing to see COVID+ and COVID PUI patients
- Required health documents include TB, MMR, Varicella, HepB, TDap, flu, drug screen, and physical
