Behavioral Health Care Advocate - Athens, OH

As a Behavioral Health Care Advocate (Athens, OH), you will serve as a clinician who visits Medicaid members in homes, community centers, hospitals, or provider offices to provide a continuum of care, medication reviews, education, and facilitate transitions from care facilities back to the home. This role is based out of the employee's home yet will be in the field approximately 10-20% of the time. If you are located in Athens, OH, you will have the flexibility to work remotely. Primary Responsibilities Visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or provider offices Provide a complete continuum of quality care through close communication with members via in‑person or on‑phone interaction Support members with condition education, medication reviews, and connections to resources such as Home Health Aides or Meals on Wheels Assist members with the transition from a care facility back to their home Required Qualifications Independently licensed clinician, with a Master’s degree in Psychology, Social Work, Counseling, or related behavioral health discipline Licenses must be independent, active, and unrestricted in the state of Ohio 2+ years of post‑licensure experience in a related mental health environment Proficient computer skills and good working knowledge of Microsoft Word and Outlook Access to high‑speed internet and a dedicated, distraction‑free workspace at home Reliable transportation and the ability to travel within the Athens, OH area Reside in or near Athens, OH Preferred Qualifications Experience working with the Medicaid population Community Mental Health experience Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients Experience working with low‑income populations Experience working with either Medicaid or Medicare funded programs Medical / Behavioral setting experience (i.e., hospital, managed care organization, or joint medical / behavioral outpatient practice) Dual diagnosis experience with mental health and substance abuse Clinical training experience Pay & Benefits The salary for this role will range from $60,200 to $107,400 annually based on full‑time employment.
